Complete the Nursing Request for Continuing Education Conference Form (Can be picked up at the Nursing Office or an electronic copy below)
Submit to the Nurse Manager/Designee for approval
Scan and email the completed form and a copy of the educational program to the finance office (RNeducationreimb@montefiore.org)

Requesting Conference Fees Pre-Pay

Finance office requires 8-10 weeks advance notice with completed documentation if requesting conference fees are paid in advance

Excuse the poor quality of the documents, that was the best version the office had available

Post conference, the nurse submits the documentation of Continuing Education Conference Expenses form to the finance office
Part A must be completed by the nurse
Attach all receipts, proof of conference attendance and CEU certificate
Part B is completed by the finance office and submitted for payment
